In the settlement Voronovo April 17, was opened at the Moscow clinical center for infectious diseases. This large-scale (about 80 thousand square meters) and ambitious project, the city authorities have implemented from scratch for almost a month.

the New clinic was built using prefabricated structures. But it’s not just a temporary hospital for pandemic coronavirus infection. This is a major clinic that will serve the city for decades. When the epidemic ends, in the Voronovo will operate modern and well-equipped infectious diseases hospital.

the Moscow clinical center of infectious diseases “Raven” is providing all types of medical care. He became part of the city clinical hospital named VP Demikhova.

the New hospital is gradually increasing its power. Today it treats more than 200 patients with coronavirus infection, a day approximately two dozen people.

Chief physician of city clinical hospital named VP Demikhova Sergei Transitions told mos.ru how the clinic as a new team and what the future holds for the medical institution after the end of the pandemic.

Sergey, as organized clinical center of infectious diseases and what are its main features?

Our medical center covers an area of over 40 hectares, and it. Of these, slightly less than half is bed resuscitation. In addition to four intensive care units, the center has seven infectious disease wards. Each — for 80 beds, in other words, we have 560 beds for the treatment of infectious patients. Chamber in the infectious ward can accommodate two to three people.

the Whole area of the centre divided into two zones: “clean” and “dirty”. In “pure” has a warehouse, security units and hostels for more than a thousand places for the medical staff and technical staff, protection.

What is the availability of hostels? After hard work our experts have the possibility not to go home and rest here.

the center organized a completely new logistics of medical care of infectious patients. The design, by the way, was engaged domestic experts, but taking into account foreign experience.

a Distinctive feature — the treatment of infectious patients in isolation to exclude nosocomial infection. The ambulance brings the patient to the emergency Department, it is placed in a special box, where the survey is conducted, documented history of the disease. And after assessing his General condition on our ambulances it’s transferred to the medical corps.

How are things with technical equipment? If there is enough artificial veinstilesii of lungs on such a large bed Fund?

In the center of a powerful diagnostic base: two computed tomography and magnetic resonance imaging. They are located in the diagnostic unit near the intensive care units. This is useful if you imported as patients from intensive care unit via separate entrances, and from infectious disease wards.

I Want to underline: we have a large number of ventilators more than 200. By and large each intensive care bed is equipped with a ventilator, and modern tracking equipment. In addition, the centre uses various methods neorganic detox — in particular, the method of extracorporeal membrane oxygenation. He has not yet been applied, the testimony was not, but we are ready to apply this method.

in addition, the center has a large lab Department with a capacity up to 10 thousand tests per day. Full deployment can be performed more than three thousand PCR tests.

we Have our own oxygen station, which provides almost all the departments. I must say that in the medical units, medical buildings to each bed is oxygen and, if necessary, the deployment of the it intensive care beds.

How difficult was it to organize the work in such a huge center, and in such a short time?

of Course, difficult, because the center is new. I and my deputies came here when the corps began to build. And for a short interval almost a month, the center was built. Worked here more than 10 thousand people around the clock.

Difficulties in the initial phase was. We were supposed to plan their actions in the unfinished buildings, when there was placed equipment. When everything is in place as in any hospital, have the equipment, experts, you just come and do the treatment. Here there was nothing. So absolutely everything from recruitment, medical equipment, its placement and adjustment, not to mention the construction and commissioning work was done fairly quickly, in unison with the builders.

I would like to say a few words of gratitude to the builders: they are for a short period of time has built this Grand structure, which I hope will work for decades. Some have sometimes the impression that the center is disposable and is constructed only for the treatment of coronavirus infection. But it is not so: the principle of construction of this center is suitable for the treatment of infectious pathologies of various diseases.

How to construct a working day the head physician of a large clinic?

I get up early enough every day at five in the morning and go to work at six. Somewhere about seven o’clock I have isus ‘ in the workplace, and my assistants at this time also already in place. I accept the reports of the duty shift. And about eight or half-past eight starts normal life: I take the Deputy for Economics, accounting. Further, when there are operations, I operate, but it is one to two times a week.

Almost the whole working day is dedicated to the organization of the medical process in the hospital. In the last month all the time spent on the organization of work in the Moscow clinical center of infectious diseases.

And how much time it takes to change the doctor at the centre?

All the doctors and medical staff work in different shifts, each with its own schedule: 24/12 hours, 24/24 hours.

How many people are already working in the state hospital?

currently employs more than 600 people. It is planned that the center will unfold at full capacity, it will employ more than a thousand people.

In the center people are all new, yet not all know each other. Nevertheless, I like the fighting spirit of our medical staff and staff support units. I am pleased to see from when a person wants something and wants to do good. It is always a pleasure to guide.

All employees are recruited from a variety of places. Therefore, training on modern medical equipment is our priority. This is especially true of service intensive care, where there are modern ventilators and monitors. We spend almost every day training. This works in a hospital simulation center.

As in your center to ensure the safety of doctors and nurses?

the Protection of medical personnel is a priority in our work. We have a sufficient number of personal protective equipment for employees, and for staff.

the Center is designed so that all employees enter the territory therapeutic area only through sanitary inspection room, and in no way through other inputs. Getting from the residential area in the sanitary inspection room, they change clothes, take a shower and undergo sanitary treatment and put on special clothes of a certain color. After that, the staff of the center are the territory, each to the medical unit.

the territory has a control tower, you can say heart of this center, which receives information about functioning of all offices and divisions. There is a control unit with a unique ventilation system that allows virtually the entire contaminated air to be evacuated through gateways and subsequently to decontaminate.

How many patients arrive at the center on an average day?

on average, we receive 20 people. Now in the center are treated only patients with COVID-19 and pneumonia.

withstanding on 4 Jun filled with 213 beds. While the number of patients with COVID-19, and pneumonia remains the same. Over a month we have treated nearly a thousand people.

How long is the treatment of a patient with coronavirus?

the length of treatment depends on the severity of the condition. The center receives patients of severe and moderate severity, the lungs of the patients here are not treated.

Initially, the structure of the center and the large resuscitation service involves the treatment of severe patients. The average day do about 20 people, and the severity of the condition, of course, due to the duration of the disease. For treatment are about 40 percent of patients in serious condition. Some are on ventilators.

What habits and illness earlier give special complications on the body by coronavirus infection?

In fact, any concomitant severe disease, especially elderly patients, are aggravated under the influence of the virus. This is the main problem.

According to our observations, in recent years there is a tendency to increase the number of cases among the younger population. What is the reason, hard to say. Maybe the beginning of self-healing can be delayed access to a doctor, but we have the trend that young people are quite seriously sick of severe viral pneumonia.

How many now prescribe patients a day?

the day we have about 15 of patients discharged. Cases of coronavirus can recover up to two weeks, but the recovery period can last for several weeks.

I Want to add that we at the center have all the features — like medicines and various treatment techniques in order fast enough to treat such a pretty severe pathology.

does the clinical center with the other medical institutions of Moscow?

first, we help all the departments and of the government of Moscow and the Department of health of the city.

We have been in contact with other agencies. The interaction between hospitals, between health institutions that are engaged in the treatment COVID-19, we are almost around the clock. Any promising positive techniques that are used in the treatment of coronavirus, apply with us.

a serious doctors and medical facilities. How do you see the future of the hospital for infectious diseases after the end of the pandemic?

the Hospital will continue to be used directly for its purpose — for the treatment of patients with various infectious diseases, for example respiratory or intestinal. I’m sure aboutand will work for many decades and operate quite successfully. All the prerequisites for that.

Sergey, how did you decide to become a doctor?

I never had a doubt that I’ll be a doctor, although in my family, no one doctor was not — neither the parents nor my grandmother or grandfather. Somewhere in the 1960-ies, when operated on dad, I first felt the smell of medical glue, and since it seemed to me that the medical profession is my calling. After school I had no doubt where to go — of course, in the medical Institute.

being a doctor is my profession. In the past I was a military doctor, and after almost 30 years in the Armed forces I retired as chief surgeon of the Ministry of defence and came to work in the health Department of the city of Moscow. For me this profession is not only a surgeon, but also a doctor of the organizer. This is my life.

How can your family relate to your work during a pandemic? What precautions do you keep coming back home?

I Have military family, so my wife and my children know where I am and what I do. They are not used to such a situation.

But precautions is quite simple: it’s basic personal hygiene. Treatment of hands with antiseptic skin, wearing masks, isolation mode for all. For us, it is not yet possible, because the work of physicians involves just work with patients. Nevertheless, I believe that all the inhabitants of our country it is necessary to observe elementary rules that require sanitary-epidemiological situation.

the restrictions were introduced in Moscow and the Moscow region, I think, allowed to go on reducing the incidence of coronavirus and prevent negative developments.

our society has finally realized the importance and heroism of doctors. You notice the attention and gratitude of the people?

I work as a doctor for over 35 years. And I notice the gratitude of patients to doctors on a daily basis.

Another issue is that today, amid such a huge number of cases of gratitude must be expressed to a greater extent. I am very pleased, when moving through Moscow, we do not see the brochures show or some shows, and see the faces of the doctors and nurses who selflessly day and night working on their jobs.
