the Pandemic has made significant adjustments to the assistance to patients with oncological diseases, which are known, are one of the leading causes of death worldwide. Many countries turned the program early screening of the population and underwent chemotherapy for patients. Chief oncologist of Russia, General Director of the National medical research centre (SMRC) radiology Ministry of health of Russia Andrey Kaprin said at the roundtable dedicated to the provision of oncotomy during a pandemic, the situation with the temporary suspension of early diagnosis of cancer will result in an increase in the number of late stages of the disease in the near future.

According to forecasts of the world Health Organization, the number of cancer diseases in the world by 2030 will double. However, the pandemic essentially crippled the entire industry: across the world urgently redeveloped cancer hospitals, delayed diagnosis and treatment patients who play for them is crucial. “For all countries, the situation was unexpected, but our oncologists never stopped working for even a day, says Dr. Kaprin. – Through Associations. oncologists of Russia we held regular teleconferences and liaising with colleagues. And we had benefits. At the time we called to close specialized clinics and centers of specialized care to cancer patients and to transfer these patients in General hospitals, but we managed to defend Oncology. So we were able to withstand the impact of the pandemic. Many hospitals in the country have kept the cancer profile and become Islands of safety for cancer patients. However, we had to turn a screening program for early detection of cancer. Therefore, it is expected that by the end of the epidemic identify more patients at advanced stages”.

According to Kaprina, significant support in pandeni was the possibility of telemedicine: “Pandemic was the impetus for the development of these technologies. We have developed the skill of remote epicrises, conduct remote consultations with scans of the patient. Almost all regions have devices for data transfer almost everywhere have developed the skills of remote analysis. By the way, our ancestors during a pandemic, the Spanish flu is also treated cancer patients, but they didn’t give us recommendations. And we will leave a guide to posterity. In addition, we plan to organize a special Biobank, which will make the data behave as anticancer drugs on the background of the coronavirus and analyse deferred effects COVID, including the oncological processes”.

As said General Director of SMRC Hematology Ministry of health of Russia Valery Savchenko, COVID, much has changed in the provision of incommodi: “He found a weak the links in those places where we are not particularly expected. The Soviet health care system was much more tenacious, and we, thank God, not destroyed, many institutions that were destroyed in the countries of Western Europe and especially the United States. And it played a key role in combating the epidemic. I’m not always sure that the creation of COVID centers is the only solution. There are other agencies that are not closed, where there is no “red zone”, but who need a means of countering the transmission of highly contagious infections. We, for example, this problem solved, bought the tests themselves, and once in three days all was in control. In the end we have no media COVID. I believe that the test in all health facilities needs, including nurses, cooks and maintenance employees, they are not always able to maintain the required standards of conduct. Test all, not stopping, we are still two to three months to prevent infection in offices, where defenceless people.”

Meanwhile, today is a very difficult situation with drug provision of the oncohematologic patients. “There is a strange phenomenon: effective basic, but cheap, drugs suddenly disappear from the market, and a significant percentage of our patients remains, I emphasize that the basic therapy. It is not even about the original products, but about cheap, but working generics. Pricing system for medicines operating in our country, discourages producers, domestic and foreign, so the drugs disappear. To replace them is very good, but expensive, not very helpful: it requires time, and changes in treatment regimens of patients for whom we, hematologists, responsible”, – said Valeriy Savchenko.

Problems with drug provision of patients with hematologic diagnoses, including children, were there, says CEO SMRC pediatric Hematology, Oncology and immunology. Dmitry Rogachev Alexander Rumyantsev: “In a country of 4.5 million patients-children, they are treated in 86 hospitals. At the beginning of the year, we announced the 10 lack of drugs, some of which are fairly simple but included in the basic protocols of patient care. The situation then intervened the Prime Minister of Russia, some positions have been filled. But not all. The situation with simple drugs is quite complex. We made recommendations to legislators and Executive authorities on the revision of the law on “the third wheel”, which does not allow to participate in trades with foreign drugs if there are two domestic ones. Many leave the market just because of the fact that this situation exists. And we have the recommendations of the FAS that the purchase of medicines must be at a minimum price – this position should be eliminated. And finally, La certain types of diseases should be allowed to buy drugs by trade names and not by international nonproprietary name, as it is established now. We were able to achieve 85% survival rate in children, but the provision of medicines is a cornerstone in achieving effective treatment.”

so far, reaction from legislators to the suggestions there, but as said the Deputy of the state Duma Valery Lakomov, regions recommended in the program of state guarantees the provision of medical care to include the provision of cancer patients with drugs at the outpatient stage at the expense of means OMS.
